Many states in the US have specialized forms for drafting a living will. In every type of form, there are certain fundamental aspects that need to be included in a living will to make it valid.
Date, month and year when the living will is drafted should be stated clearly.
Identifying yourself: One should include name, social security number and communication address.
Age and legal status: A person writing a living will should be at least 18 years old and be a legal resident of the region.
Attestation: One should provide the names and signatures of two witnesses along with their addresses. The witnesses may or may not be related with the person by blood or marriage.
Current medical condition: Person should provide details about his existing medical condition. This can be written in consultation with a family health care provider.
Next important aspect is to list out a set of particular medical scenarios where the living will has to be considered. Options are listed out on the form. One needs to select from the list.
One has to specify the goals of their medical care in each scenario from the list. For example, the patient might want to just prolong his life in every possible way, or just want to have only comfort care or wants to limit medical treatments to less invasive methods.
A declaration should be made by the person that if any time a medical complication provided in the list arises, he would like to treatment to go in a specified way. Specifications should be clearly described by the person.
